This free skin cancer spot check is provided by a qualified medical professional for anyone concerned about certain spots on their skin. Its purpose is to raise general awareness about your skin and skin cancer, so a diagnosis will not be provided.
We check one to three spots. Because you are having only certain spots looked at, we recommend you also arrange a full body skin check. This is not a dermatology consultation, and other skin issues will not be addressed.
The medical professional cannot offer ongoing management of anything they find, though we may contact you about a particularly urgent concern. It is essential that you are registered with a GP: if a concerning spot is identified you will need to make an appointment with them, and making that appointment is yours to do.
Information collected on this form and during the check is handled under the Privacy Act 2020 and the Health Information Privacy Code 2020.
It is held by Melanoma New Zealand and its service providers to run the spot check and talk to you, and to your GP, about it. That may include sending images taken during the check to you or to your GP. It is also used to send you general updates if you ask for them.
You do not have to give us this information, but without it we may not be able to give you a spot check. You can ask to see or correct what we hold at any time.
